This CAD image shows how both the intake and exhaust cam of the Aprilia Shiver engine (a Piaggio Group design, not Rotax) are driven by a single gear, itself driven by a chain. This allows the exhaust cam to be located lower, providing extra clearance for the radiator and front wheel.  View Related Article
